Your User Name should consist of: --the first three letters of your last name --the first three letters of your first name --your two-digit day of birth (For a person born on January 3, 1960, the two-digit day of birth would be 03). Example: For Pat Jones, Birthday 01/03/60, User Name would be JONPAT03 Click the button marked "Login to Survey." HOW TO SAVE YOUR WORK AND SUBMIT YOUR COMPLETED SURVEY You will be prompted to save your work at regular intervals. A prompt will appear on the screen instructing you to click a button marked "Submit Items." By clicking the "Submit Items" button, your answers will be recorded and you will be transferred automatically to the next set of items. Upon completion of the entire survey, a message will appear that says, "You have now completed all survey sections." HOW TO EXIT AN INCOMPLETE SURVEY If you cannot finish the entire survey at one sitting, exit only when you have responded to all items in the section you have begun. This will ensure that your work is saved. Return to the Web site http://www.websurveyor.com/chauncey/g10 Enter your User Name in the User Name box. Click the button marked "Login to Survey." You will be transferred automatically to the first survey section you did not complete. Note: If you re-access your survey on a different computer, you may be transferred to Section 1 of the survey instead of the first section you have not completed. If this happens, proceed as follows. Scroll to the bottom of each survey section you have completed. Click the "Submit Items" button at the end of each section. Proceed through the survey until you locate the survey section you have not completed. Then begin to make your ratings. You do not need to re-enter your ratings for sections you have completed.
